Hope House and Tŷ Gobaith have experienced children’s nurses who provide nursing within the family home. Their aim is to provide help and support within the home when a family needs it most. Often this will be some kind of crisis, or when a child/young person becomes particularly unwell.
The Outreach Nurses can give practical nursing care for several hours or a whole day or night at a time. Support is also offered if a child/young person is in hospital for a prolonged length of time. The nurse may stay with the child/young person on the ward/unit, allowing the parents to spend some time away from the hospital environment. Experienced Care Assistants may provide additional support to families as part of the outreach service.
Should the family choose to stay at home during the end-of-life stage of illness, then in conjunction with other professionals involved, the Outreach Team can assist them to fulfil their wishes.
Any family accepted for support from Hope House has access to the outreach service and requests for help will be taken from anyone involved in their care.
