The Role

Hope House Children’s Hospices is a centre of excellence for children’s palliative care ensuring every family facing the death of a child gets the very best care and support when and where they need it.

We are recruiting for an effective leader to manage and lead a multi-disciplinary service delivering high quality, seamless services to children and families  from referral through to discharge.

Reporting to the Head of Care, you’ll first of all be passionate about wanting to help local children and families. Your role as Pathways Manager is vital to ensure equality of access and good introduction to the service for all children and families. Your team will maintain relationships with families through our Key Worker Service and prepare children and families to transition into adult services with confidence.

You will be a registered health professional utilising your excellent communication, leadership and people skills to work closely with other leaders in care and external partners to manage the following key pathways, Neonatal, Transition and Bereavement, and our Key Worker and Social Work services.
