Our Criteria

For Care:

The Hope House and Tŷ Gobaith Referrals Panel is made up of the hospice medical officer, senior nurses and social work staff who decide whether or not a referral is accepted. They need to see that, in the opinion of medical professionals involved, the child or young person is not expected to live beyond early adulthood (25 years).

We care for babies, children, young people and young adults who live in Shropshire, Cheshire, Mid and North Wales with a range of conditions:

Group 1 - Life-threatening conditions for which curative treatment may be feasible but can fail, where access to palliative care services may be necessary when treatment fails. Children in long-term remission or following successful curative treatment are not included. (Examples: cancer, irreversible organ failures of heart, kidney, liver)

Group 2 - Conditions where premature death is inevitable, where there may be long periods of intensive treatment aimed at prolonging life and allowing participation in normal activities. (Examples: cystic fibrosis)

Group 3 - Progressive conditions without curative treatment options, where treatment is exclusively palliative and may commonly extend over many years. (Examples: Batten Disease, Mucopolysaccharidoses, Muscular Dystrophy)

Group 4 - Irreversible but non-progressive conditions causing severe disability leading to susceptibility to health complications and likelihood of premature death. (Examples: severe cerebral palsy, multiple disabilities, such as following brain or spinal cord injury)

For more information please email [email protected] or call Hope House on 01691 671999 or email [email protected] or call Tŷ Gobaith on 01492 651900.

For Immediate Care After Death:

Immediate care after death is available to any family whose baby, child or young person (up to the age of 25) dies in any circumstances.

The family must live in Shropshire, Cheshire, Mid or North Wales, but do not need to have had any previous contact with Hope House Children's Hospices.

For more information please email [email protected] or call Hope House on 01691 671999 or email [email protected] or call Tŷ Gobaith on 01492 651900.

For Bereavement Support:

Counselling and bereavement support is available to anyone who has been affected by the death of a baby, child or young person (up to the age of 25 years when they died). Their death may have occurred in any circumstances.

This support is available to children, young people and adults who live in Shropshire, Cheshire, Mid or North Wales. You do not need to have had any prior contact with Hope House Children’s Hospices.

For more information please email [email protected] or call the Sunstone Centre on 01691 672618 or email [email protected] or call Conwy Court on 01492 554443.
