Referrals At Hope House Children’s Hospices, we provide specialist care and support for babies, children and young people with life-threatening conditions. We are here to help families make the most of every moment, offering care, comfort and understanding during some of the most difficult times. Start referral Who we support ▶ We care for children with: 1. Conditions where treatment may help, but could failChildren may need our support during treatment or if treatment stops working. 2. Conditions where a child’s life may be shorterMany children receive treatment for long periods to help them live as fully as possible. 3. Progressive conditions without a cureOngoing support is needed over many years. 4. Severe, non-progressive conditionsThese conditions may cause significant disability and lead to health complications or a shorter life expectancy. If you’re unsure whether your child is eligible, please get in touch — we are here to help and guide you. Who can make a referral ▶ Anyone can refer a child to Hope House and Tŷ Gobaith, including: Parents and family members Doctors, nurses or other healthcare professionals Social workers, schools or community teams We simply ask that the child’s parent or person with parental responsibility agrees to the referral. How referrals are assessed ▶ Once you submit a referral, our team will make contact with you to let you know that we have received it. Your referral will then be reviewed by our Referral Panel. The panel meets every two weeks and includes the hospice medical officer and senior nurses from both hospices. Each referral is considered carefully and sensitively, and assessed in line with our criteria, policies, and the care needs of your child. When a decision has been made, you will receive a letter letting you know the outcome. If you haven’t heard from us within the expected timeframe, please get in touch and we’ll be happy to update you on the progress. Information that helps with a referral ▶ Please make sure you have the following information about your child to hand when making a referral: Full name, address and family contact details Recent clinic letters or reports which confirm their diagnosis, prescribed medications and nursing needs. Their symptom management plan and/or advanced care plan (if relevant) Contact details of the medical professionals supporting your child If you don’t have everything, please don’t worry — we can support you through the process.
